Below you will find the arrangement forms we use when assisting families who do not already have prearrangements on file with us at Allen Funeral Home, along with our current General Price List outlining the goods and services we regularly provide.

The Comprehensive Arrangement Form is designed for families who would like to prepare in advance for many of the questions our funeral directors may ask during the arrangement process. The Simple Arrangement Sheet includes the basic information needed at the time of loss and may be easier to complete for families who prefer a shorter form.

If possible, we encourage you to review these documents prior to meeting with our staff and complete whichever arrangement form best fits your needs. Doing so allows our team to make the arrangement process simpler, more personalized, and more focused on honoring your loved one and supporting your family during a difficult time.
